Lower or Higher Oxygenation Targets for Acute Hypoxemic Respiratory Failure

نویسندگان

چکیده

In patients admitted to the ICU for hypoxemic respiratory failure who receive supplemental oxygen, there are conflicting data showing increased or decreased mortality in treated with a lower oxygenation target compared higher target. One meta-analysis showed targets were preferable critically ill patients, but other trials stopped early due an increase lower-oxygenation groups and increases mesenteric ischemia.

Acute hypoxemic respiratory failure following intrapleural thrombolytic therapy for hemothorax.

Intrapleural instillation of thrombolytic agents has been useful in the treatment of hemothorax when thoracostomy tube drainage is unsuccessful. We present a patient who developed acute hypoxemic respiratory failure following the intrapleural instillation of both streptokinase and urokinase 24 h apart. Hypoxemia most likely resulted from a direct effect of the products of fibrinolysis on the pu...

Supporting oxygenation in acute respiratory failure.

Strategies to support oxygenation can cause substantial harm through lung stretch injury, oxygen toxicity, transfusion risks and cardiac over-stimulation. Traditional goals of maintaining near normal cardiorespiratory parameters are most likely overly simplistic and are insensitive and nonspecific for tissue hypoxic effects. In order to reduce iatrogenic harm, it is conceivable that clinicians ...

NIV-Helmet in Severe Hypoxemic Acute Respiratory Failure

Noninvasive ventilation (NIV) is a method to be applied in acute respiratory failure, given the possibility of avoiding tracheal intubation and conventional ventilation. A previous healthy 5-month-old boy developed low-grade intermittent fever, flu-like symptoms, and dry cough for 3 days. On admission, he showed severe respiratory distress with SpO2/FiO2 ratio of 94. Subsequent evaluation ident...

Noninvasive ventilation in patients with hypoxemic acute respiratory failure.

PURPOSE OF REVIEW To discuss the recent literature concerning the use of noninvasive ventilation for hypoxemic acute respiratory failure. RECENT FINDINGS The benefits of noninvasive ventilation for patients with hypoxemic acute respiratory failure are unclear. In immunocompromised patients and following thoracic surgery, there is a strong rationale for using noninvasive ventilation to treat a...
